스테로이드 및 피부외용제의 올바른 사용법

피부발진의 국소적 치료시 급성기일수록 즉, 염증이 심하여 진물이 나는 상태, 습포 등의 수치요법을 시행하여 염증을 완화시킨 후에 다른 제제를 사용하도록 하고 만성화된 병변일 수록 연고제를 사용한다. 따라서 다음과 같이 염증의 단계에 따라 적절한 제제를 선택함이 매우 중요하다. 그리고 모발이 많은 부위에는 로숀(lotion)이나 gel 형태를 사용한다.

수치요법(특히 습포)은 다음과 같은 작용을 갖는다.
1. 청결작용
2. 항소양 및 진통작용
3. 항소염작용
4. 연화작용
5. 건조작용
상기와 같이 병변의 단계 혹은 상태에 따라 적절한 제제를 선택하기 위하여 피부 외용제 의 기제의 분류 및 종류에 대하여 알아둘 필요가 있다. 그리고 국소제의 일반적인 도포 방 법을 알아보고 흔히 사용되는 국소 스테로이드제의 분류 및 유의점에 대하여 언급하기로 한 다.
A. 기제(vehicle)의 분류
1. monophasic
1) 분말 (powder):
-일반적으로 건조하게 한다.
-zinc oxide, titanium oxide, talc(magnesium silicate)
-간찰 부위에 진물이 있는 상태에서는 사용하지 말아야 한다. 삼출액과 섞여 덩어리를 형성 하여 자극을 줄 수 있기 때문이다.
2) 액체 (liquid)
(1) 습포 (wet dressing):
-급성 염증시 사용(oozing, weeping, crusting)
-Burow sol (aluminium acetate); astringent to decrease exudation by precipitation of protein
-potassium permanganate(KMnO4, 1:4,000-1:10,000); 항균, 항진균 작용이 있음
(2) 목욕 (bath):
-광범위하게 삼출액이 있는 경우에 사용
-장기간 지속하면 침연(maceration)을 일으키므로 목욕시간은 20-30분을 초과하지 않도록 한다.
-starch added for soothing and antipruritic effect
-bath oils added to prevent drying
(3) tincture, paint : iodine tincture, Castellani paint(과도한 건조와 붉은 착색으로 현재는 거의 사용 한함)
(4) 용액 혹은 로숀 (solution or lotion)
-용액(active ingredients are dissolved to clarity), 로숀(active medications suspended or dissolved): 액상이 증발하면서 병변의 건조와 냉각을 유발한다. 따라서 진물이 있는 정도의 아급성 병변에 사용하거나 전신의 광범위한 피부병변에 도포하기 쉽도록 사용한다.
-염증의 정도가 심해서 직접 도포가 힘들거나 동통을 유발하는 경우에는 aerosol 혹은 spray 형태로 사용이 가능하다.
(5) 겔 (gel) (transparent, colloidal dispersion that liquefy on contact with skin)
-nonocclusive film형태로 건조되고 모발이 있는 부위에 사용이 적절하다.
3) greasy bases
(1) 기름 (oil): 피부로부터 기름과 섞이는 제제를 제거하기 위하여 사용하거나 목욕시 연화 제로 사용하기 위하여 첨가한다.
(2) petrolatum(vaseline): emollient, highly occlusive and water-insoluble and hydrates skin by preventing water loss
2. Biphasic vehicles
1) shake lotion (로숀에 분말이 혼합된 것):
-zinc, talcum, calamine--dry and cool wet, weeping skin
-균일한 현탁액(suspension)을 만들기 위하여 사용전 반드시 흔들어 준다. 피부에서 액상이 증발되면 분말이 남는다.
2) 크림 (cream):
-emulsion of oil-in-water (O/W)--cooling, moisturizing, emollient effects
3) 연고 (ointment):
water-repellent hydrocarbon(petrolatum)
absorption bases(hydrophilic petrolatum)
water-in-oil (W/O) emulsion(hydrophilic ointment)
water-soluble ointments(polyethylene glycol preparation)
-크림제보다 피부에 보호막을 형성하고 윤활 효과가 크다.
4) 이고 (paste)(ointment+ 20-50% powder): zinc oxide
-연고에 비해 덜 침연을 일으키고 삼출물을 흡수하는 경향이 있으므로 연고를 사용할 수 없는 아급성 병변에 사용할 수 있으나 근래에는 거의 사용하지 않는다.
B. 흔히 사용되는 기제 성분 (vehicle ingredients commonly used in topical preparations)
1. 연화제 (emollients):
-occlusive agents that make skin soft and pliable by increasing hydration of stratum corneum
-petrolatum is the most occlusive and the best emollient--due to its greasy feel, petrolatum often mixed with other material to produce a more cosmetically acceptable vehicle.

4. 안정제 (stabilizers):
-preservative(prevent microbial growth, paraben)
-antioxidant(drug or vehicle may deteriorate through oxidation--butylated hydroxyanisole in oil and fats, ascorbic acid, sulfites in aqueous phase)
-chelating agents(to compex heavy metals, citric acid and sodium EDTA)

C. 국소제의 도포시 고려사항
1. 농도 (concentration): 어느 정도 농도 이상이 되면 더 이상 효과가 증대되지 않는다.
2. 도포회수
일반적으로 하루 2-3회 도포하나 아주 강한 스테로이드제의 경우에는 하루 1회로 충분하다.
3. 도포량
1 g이 약 10 x 10 cm 정도의 부위를 덮는다. (예, face or hands-2g, 1 arm or anterior or posterior trunk-3g, 1 leg-4g, entire body- 30g)
4. 신체부위에 따른 투과의 차이: 다음과 같은 순서로 약제의 투과성이 감소한다.
1) 점막 (mucous membrane)
2) 고환 (scrotum)
3) 안검 (eyelid)
4) 얼굴 (face)
5) 흉부와 배부 (chest and back)
6) 팔과 다리 (upper arms and legs)
7) 손등, 발등 (dorsa of hands and feet)
9) 손, 발바닥 (palmar and plantar skin)
10) 조갑 (nails)
5. 피부장벽 (skin barrier)
1) 각질층 (stratum corneum): 10-20 m의 두께
2) 세포간 지질 (interceulluar lipid) (ceramide, cholesterol, long chain fatty acid); diffusion barrier
3) 피부부속기 (appendage): 0.1%-1% of skin area 장벽의 손상이 생기면 흡수가 증가된다.
D. 국소 스테로이드제
1. 분류
처음 1-2주간은 강한 제제로 도포를 시작하고, 그 다음에는 처음제제보다 다소 약한 제 제로 3-4주간 사용한 다음 호전된 이후 hydrocortisone 제의 약한 제제로 유지하는 것이 좋 다. 환부의 부위별(얼굴 및 성기부위, 구간과 사지, 손발 등의 크게 3부위 정도로 구분)로 적 당한 강도의 스테로이드제를 선택하여 사용하며, 참고로 국소 스테로이드제의 강도에 따른 분류는 다음과 같다.

2. 부위별 외용 스테로이드제의 강도
1) face and genitalia: weakest
2) trunk, U/E, L/E: intermediate
3) hands, feet and scalp: strongest
3. 임상적인 유의점
1) 소아의 경우 체표면적이 성인에 비하여 상대적으로 크고, 대사능력이 자고, 피부가 얇기 때문에 흡수가 증가된다. 노인의 경우에도 피부가 얇기 때문에 흡수가 증가된다.
2) 임신중 사용: 동물실험에서 강한 스테로이드제를 과량 도포했을 때 태아의 기형을 유발 할 수 있다는 보고가 있다. 인체에서는 임신 중 전신적으로 스테로이드를 복용하여도 태아 기형이 증가한다는 보고는 없지만 임신 중 주의를 요하는 약으로 FDA에서는 C군으로 스테 로이드를 분류하고 있다.
4. 부작용
1) epidermal thinning--ulceration
2) reduced dermal collagen--telangiectasia, purpura, aging
3) reduced dermal elasticity--striae
4) increased sebaceous activity--acne
5) hair follicles stimulated--hirsuitism
6) reduced immune response: infection exacerbated, and masked
